Resistance to therapy and chronicity are to be expected in almost half of patients with mental illnesses. To avoid stigmatization (.... has someone "resisted" good therapy?, or does the "failure" experienced by us practitioners prevent the necessary intensification or specification of our therapeutic efforts?), we should pay more attention to the important "pathophysiological" factors of chronification.
