The "eHealth-supported case management for mentally ill patients in primary care" project for patients with depression and anxiety disorders funded by the Innovation Fund (coordinated by Dr. Anne Moschner, TK Hamburg) is investigating a case management program in Hesse that is carried out by a team of GPs and medical assistants with the patient using e-health tools.
