Reducing medication problems and drug interactions

Health problems that can arise from taking different medications incorrectly are often underestimated. This can lead to repeated hospital admissions and an increased need for therapy. Older people, who often have to take several medications at the same time, are particularly affected. In the INTERPOLAR use case, the four consortia of the medical informatics initiative are working on developing IT solutions to improve drug therapy safety. The INTERPOLAR use case aims to use an algorithm to identify hospital patients who have a high risk of clinically relevant and controllable medication problems.

This allows hospital pharmacists to focus on the patients who will benefit most from their assessment. Risk triggers selected by experts provide hospital pharmacists with targeted support in finding patients with an increased risk of medication problems. This can speed up risk assessment in routine evaluation and focus the medication analysis for patients at risk. Accompanying studies should show that IT-supported risk assessment significantly reduces the number of medication incidents.
